Student Record Sheet

A student record sheet is where the teacher records information from the assessment.
It is four pages long and provides a lot of information about the student's reading abilities.
It includes a set of checklists for indications of whether a students is able to retell what they read.
A copy of the text is on the sheet, where teachers mark down any errors and self-corrections as the student reads and takes note of reading strategies used by the student.
Comprehension questions and their possible answers are provided, where the teacher checks if the question is answered correctly or not.
Equations on how to figure out accuracy are laid out and recorded on the sheet as well as a fluency checklist.
Gathering all the information together, teachers are able to figure out where the student is in their reading abilities.
A place for writing down recommendations to help improve reading is filled in by teacher and discussed with the student.
